Help navigate and connect older adults to resources and services through information and referral services including food and nutrition, utility assistance, support groups, healthcare and benefits, legal aid services, housing and other vital safety net services.
You will:
Provide resources in a professional, nonjudgmental, culturally-appropriate manner
Identify the needs of older adults and refer to appropriate resources
Confirm resources are current and accurate
Provide lanugage support to community partners
